Dog of Distinction Entry Form

By filling out and submitting this form, you agree to permit Northfield Dog Training (NDT) to publish your name, your dog's name, and the title's you've earned. You also agree that any picture you provide is owned by you and not copyrighted by some other individual or organization. Once you submit your form, NDT will review the information you provide and make it available, at NDT's sole discretion, on the Dogs of Distinction page. We will endeavor to review submissions as quickly as possible. NDT takes no responsibility for the accuracy or completeness of the information displayed on the Dogs of Distinction page. If you find an error, please let us know.

Note: If you need to edit your entry after approval, you must enter your last name and your dog's call name exactly as you entered it the first time. You may come back and enter additional titles, add a picture, or change your dog's registered name. You can include titles with your dog's registered name. The maximum number of characters in a registered name is 254.
